Are foreigners overcharged at Korean clinics?
Korean patients and foreign patients are often quoted very different prices for the same procedure. Here is where the gap comes from, and how to close it.
The gap is real, and it is mostly structural
Korean cosmetic clinics run two very different sales channels. Domestic patients book through apps like Babitalk and Gangnam Unni, where clinics compete openly on price and publish discounted event rates. Foreign patients usually arrive through an agency, a hospital's international desk, or a package deal — channels that carry commission, interpreting, coordination and marketing costs. The surgery can be identical while the invoice is not.
What Korean patients actually pay
We read 337,467 Korean-language reviews and pulled out every figure a patient stated they had paid. For rhinoplasty, 184 patients across 45 clinics reported a median of about ₩4.4M, with the middle half falling between ₩3.0M and ₩5.6M. For eye surgery, 269 patients across 73 clinics reported a median near ₩1.31M. These are totals patients said they paid, not advertised starting prices.
Why the advertised price is always lower
The number in a clinic's promotion is a floor, not a forecast. It usually covers one item at the simplest technique, excludes anaesthesia beyond local, excludes implants or cartilage grafts, and assumes no revision history. A first-time closed rhinoplasty and a revision using rib cartilage are both 'rhinoplasty' in an ad and are not close in price.
Agency commission is not disclosed to you
Medical tourism agencies are typically paid by the clinic as a percentage of your bill. That is legal and common, and it also means the person helping you choose has a financial interest in where you land and how much you spend. An agency can still be worth using for logistics. Just do not treat its shortlist as neutral research.
How to close the gap in practice
Ask for an itemized written quote before you fly, listing the surgical fee, anaesthesia type and fee, implant or graft materials, facility fee, follow-up visits, and what a revision would cost. Ask what the same procedure costs a Korean patient booking directly. Get quotes from at least three clinics. Bring the itemized quote to your in-person consultation and confirm nothing has changed before you pay a deposit.
When paying more is the right call
A higher price is worth it when it buys a board-certified specialist performing your surgery personally, a real anaesthesiologist for anything under sedation or general, an accredited facility, and a written follow-up plan you can use after you fly home. It is not worth it when it only buys a nicer waiting room and a driver.
